- Same Appointments – No need to reschedule. Your date, time, and provider remain the same.

A member from our team will contact you to help you complete the new consent forms and set up your new patient portal account.
Patients of Miguel Diaz
If you have not been transferred to a new provider already, you will remain with him as your provider. Everything will remain the same and you do not need to sign any new forms.
Patients of Gabriel Rivera Torres or Barbaro Sarosa Cepero
- If you live in the Cape Coral area you will need to complete new forms and set up a new patient portal account with FHS Cape Coral.
- If you live in the Miami/Doral area you will need to complete new forms and set up a new patient portal account FHS Doral.
ALL OTHER PATIENTS
- You will need to complete new forms and set up a new patient portal account with FHS Cape Coral.

Want To Speak To Your Provider?
If you need to contact your provider, you must send a message through the FHS patient portal. Providers will no longer receive messages sent through the CBH patient portal.
Pay Your CBH Bill
If you need to pay your bill for appointments that were completed before April 30, 2025, you must log into the Community Behavioral Portal, NOT the new Florida Healthcare System portal. Although the portals look very similar, they are two different accounts. Use the link below to pay for a bill with Community Behavioral Health.
