If you are looking to become a new patient at Community Behavioral Health you must call our office to start the application process. All applications will be carefully reviewed to make sure that we are the right fit for each other. After your application has been approved and you have successfully completed your intake forms, we will schedule you with one of our providers.

You will complete a short and simple intake process that should take no longer than 5 minutes.
After your appointment is scheduled, you must complete new patient forms before your first appointment. Failure to complete forms may cause a delay in being seen by a provider.
Please have ready before you call to schedule an appointment:
1) Your insurance card
2) Medication list
3) Medical Records (if available)
If you fail to bring in any of these items, your appointment may need to be rescheduled. We look forward to serving you.

The fastest way to request a refill is by logging into your Patient Portal and clicking on the “Prescriptions” button at the top of the page.
Please note that CBH does not process emergency refills. Refills are only processed Monday-Wednesday. Refills must be requested at least 1 week before you run out of medications. Please check when your prescription expires and plan accordingly. Call your pharmacy to check on the status of your prescription before making any refill requests to our office.
Log into your Patient Portal. Click on the “Messages” button in the menu at the top of the page. When you’re on the Messages page, click “Ask A Question”. Complete the form with your request and click “Send Message”.
Please allow up to 48 hours for your provider to respond to your request.
CBH has strict late cancelations and no-show policy. You must cancel or reschedule at least 24 hours before your appointment or you will be charged a late cancelation or no-show fee that must be paid before you can schedule your next appointment.
